Download and complete this HEALTH HISTORY FORM prior to your visit. This sheet is designed to help you get the most out of your treatment. Also, please read our Disclosure Statement. Please read it through carefully - it will answer many of your questions!

Before Your Treatment:

Wear comfortable clothes that can be easily removed or slid along your limbs. During a treatment we might need to access various parts of your body, usually, arms, legs, abdomen and back.

Eat something light an hour before your treatment so that you will not arrive famished. Acupuncture utilizes your body's energy much like an exercise session would, and if your blood sugar is low, you will not respond as quickly to the acupuncture treatments.
Avoid heavy meals or alcohol prior to your appointment - these can interfere with both the diagnosis and treatment.

Avoid sex and vigorous exercise for at least one hour before treatment. These activities can mask important diagnostic indicators that are important for making an accurate diagnosis.

Please refrain from wearing oils, lotions, and strong perfumes or colognes the day of your treatment. Again, they can mask important diagnostic indicators. Plus, many of our patients are being treated for allergies and chemical sensitivities.

Avoid receiving massage, chiropractic adjustment or other body work the day of your treatment.

After Your Treatment:

Plan to rest and relax after your treatment. If at all possible, eat lightly, spend time in quiet activities and let your body heal.

If you must return to an active life right away, avoid food, sex and vigorous exercise for at least one hour. Above all, avoid alcohol and heavy meals. All of these can interrupt the flow of the vital energy, which has been released during your treatment.
